The requirements vary from school to school. Some will ask you to simply provide three letters of recommendation from people other than your friends (usually college professors or work supervisors).
The required number of recommendations also varies. If the school you’re applying to only requires a couple of letters, don’t try to impress them by compiling five or six recommendations. Instead, focus on getting the two best people possible for your required letters of recommendation.
So who should you ask for a letter of recommendation? As mentioned above, find the people you are absolutely sure will provide a great recommendation. That means their letter won’t be high quality. Regardless of who you ask to write a letter of recommendation, make sure you provide them with your application.

The key to asking for a letter of recommendation for graduate school is making it as easy as possible for the person you are asking. I always offered to prepare a rough draft for them.
Tip: ask more people than you need, for example if you are trying to get three letters of recommendation, then ask five different people. With more options you can chose the top three letters to submit. The person writing the letter of recommendation will never know that you did not submit their letter.
The best letters of recommendation include specific instances of conduct and quantifiable data. When someone replies with a finished letter of recommendation it is a good idea to send a small thank you card, by way of regular snail mail indicating that you appreciate their time and the letter of recommendation.
This only applies to situations where you were approved, you would never mention that you were not approved unless you are specifically asked if you were approved for the program or school, by someone who agreed to write a letter of recommendation.
It will be easier and more effective to ask for a letter of recommendation if you remember that it is usually flattering to the person you are asking. Your going to make it as easy as possible by offering to prepare a draft letter for them to edit. You should ask more people than you need or want letters of recommendation for graduate school.
